Memphis-Mid-South Affiliate of the Susan G. Komen for the Cure Funds Community Outreach Programs

Volunteers Help Serve Unmet Breast Health Needs of the 
MidSouth’s Medically Underserved

The Memphis-MidSouth Affiliate of Susan G. Komen for the Cure, one of over 100 Affiliates nationwide dedicated to putting an end to breast cancer, recently provided community grants totaling $865,000 to support Shelby, Desoto, Tunica, Tipton and Fayette county breast health education and breast cancer screening and treatment programs.
	The Komen Memphis-MidSouth Affiliate’s local fundraising efforts, such as the Komen Memphis-MidSouth Race for the Cure®, have enabled it to fund the following projects:

Necessities, Inc.
Necessities Bag Distribution
Necessities, Inc. is a non-profit organization based in Ridgefield, CT.  The organization serves mastectomy patients and their caregivers by providing Necessities bags to women in Connecticut, California, Illinois, Texas, New York, Montana and Tennessee.  The reusable canvas tote contains a pamphlet of practical tips about managing the hospital stay and planning for homecoming along with wound care supplies and other items for personal care and hygiene.
Funded Amount:  $5,000

Tennessee Men’s Health Network
Male Breast Cancer Awareness
Tennessee Men’s Health Network will create an education campaign that provides information for men and women on male breast cancer risks, prevention, screenings and treatments.  TMHN plans to provide comprehensive educational materials, screening guidelines, referral services and knowledge of how to detect and deal with breast cancer as a man.  Will provide education materials in Shelby, Fayette, Tipton, Desoto and Tunica counties through presence at health fairs, church events, work places and other outreach activities.  A website and PSAs will also be developed.  
Funded Amount: $8, 649 

Komen’s Small Grant Pool
The Komen Memphis-MidSouth Affiliate will institute a small grant pool in which funds will be distributed to individuals and organizations directly through the Affiliate.  Guidelines will be developed and distributed to the community through the Affiliate.  All funds dispursed will be at the discretion of the Affiliate.  
Funded Amount:  $10,000

Memphis Cancer Foundation
STAAR Witness
STAAR Witness Project is an outreach activity of the STAARS group.  The project will provide (1) BSE education sessions, (2) breast health resource guides, (3) support vouchers for financially eligible breast cancer patients, (4) group information support meetings and/or educational materials, (5) mammography screenings for uninsured, health and physically challenged women and (6) survey participation for breast cancer survivors.  The project’s goal is to reach approx. 2000 women with 500 reached through formal sessions and 1500 through informal sessions.  Will also provide support vouchers for patient treatment and care services and survivor post-treatment and care services.
Funded Amount:  $30,000

Children and Family Services
Komen’s Breast Health Project
Children and Family Services will provide educational seminars, support services and community awareness on breast health to survivors and at-risk individuals in Tipton County.   The Komen Breast Health Project will provide seminars for 120 survivors or individuals with a family history of breast cancer and support group sessions for 60 survivors.

Urban Health Education and Support Services
A Carin’ Touch
A Carin’ Touch provides culturally competent services to newly diagnosed breast cancer patients, survivors, and/or relapsed women who are primarily African American and/or from economically disadvantaged backgrounds.  There are 4 components to the program: (1) a support group, (2) community outreach, (3) Going the Extra Mile program and (4) services to newly diagnosed patients.  In 2007, the organization expanded activities to include Fayette County.  In 2008, a program to address the financial issues with cancer survivorship is being added.   Over 150 women will be directly served through the program.  Over 2100 will be reached though presentations, health fairs, etc.
Funded Amount: $33,000

Tipton County Commission on Aging
Project LEAP
The Tipton County Commission on Aging proposes to advance the cause of breast cancer wellness using the Komen LEAP (Locate, Educate, Activate, and Participate) model.  Using this model, TCCA will implement the Project in two isolated areas of rural Tipton County and during the Annual Health Fair at the Covington Center.  Targeted population – women 55 years of age and older.  Transportation will be provided to the education sessions, the host site for mammograms, the follow-up luncheon and the Caregiver Wellness meetings as needed.  Will provide mammogram costs for at least 60 uninsured and/or underinsured women.
Funded Amount:  $37, 524

Desoto Health & Wellness Center
Know the Signs
The Desoto Health & Wellness Center seeks to provide education, breast cancer screening and demonstrations on self – examination to the working uninsured women of Desoto County.  The target population will receive screenings for the estimated 200 women at no cost.
Funded Amount:  $50,000

Christ Community Health Services
Care Stream
Through its Care Stream Women’s Outreach Program, CCHS seeks to provide routine screening and education services to medically underserved women in Southwest Memphis, Binghampton, Frayser and Orange Mound communities.  Targeted population:  women over the age of 50.  
Funded Amount:  $75, 300

West Tennessee Area Health Education Center
Project: WE CARE
Project: WE CARE is designed to educate and empower economically disadvantaged women and youth in Fayette and Shelby counties to adopt good health practices.  The project goals are to increase knowledge regarding breast cancer and to increase screenings and early detection.   Project: We Care will provide education to 500 women; mammogram screenings, follow-up and referral/support for 100 women ages 35-69 and education and support/referrals to 250 youth ages 14-18.  
Funded Amount:  $76, 675

Wings Cancer Foundation
Back on Track – Breast Cancer Survivorship Program
Wings will dedicate a Special Projects manager to focus on identifying and dealing with survivorship issues and to create and implement a Breast Cancer Survivorship Plan.  This plan will summarize information critical to the breast cancer survivor’s long term care, such as diagnosis, treatments, potential long term effects and the timing of follow-up visits as well as recommendations for obtaining and maintaining a healthy lifestyle.
Funded Amount:  $80,000

Church Health Center
Comprehensive Breast Care
The CHC goal is a comprehensive approach to fighting breast cancer among the working uninsured, homeless, elderly and underserved in our community.  Through the CHC clinic ministry, they provide medical services including screening mammograms, diagnostic mammograms, ultrasounds, core biopsies and other treatment as needed.  Through the CHC wellness ministry, Hope & Healing, and their Faith Communities Ministries, they provide awareness events and educational outreach.  CHC expects to serve approx. 2,500 – 3,000 women in our community.  They estimate approximately 1,000 through the clinic and 2,500 through Hope and Healing and congregations.
Funded Amount:  $85,852

Julie B. Baier Foundation
Julie B. Baier Foundation’s Diagnostic Treatment
The Julie B. Baier Foundation dba The Mroz Baier Breast Care Clinic will serve the diagnostic needs of 600 uninsured patients with a screened breast problem.  Patients will come from Tipton, Shelby and Fayette counties.   The clinic accepts referrals from the Church Health Center and Christ Community Health Center.  Services that may be performed include diagnostic mammography and its interpretation, cyst aspiration, image guided biopsy to include stereotactic biopsy.
Funded Amount: $100,000

Baptist Memorial Hospital for Women
Filling the Gap – Mammography for Underserved Women
The Filling the Gap – Mammography for Underserved Women program will help Susan G. Komen for the Cure meet its goal of providing breast cancer screenings and diagnostic services for 350 medically underserved women in Shelby, Fayette, Tipton, Desoto and Tunica counties, with a focus on African American, Hispanic and rural women in the five county area.

“In order to ensure our funds are making the greatest impact, the Komen Memphis-MidSouth Affiliate works with local medical experts and community leaders to conduct comprehensive community needs assessments,” said LaShawn Hampton, Komen Memphis-MidSouth Affiliate Grants Chair. “We have identified specific, unmet breast health needs within our community and ‘filled in the gaps,’ delivering the life-saving message of early detection and providing assistance to medically-underserved breast cancer patients and their families.”
	Up to 75 percent of funds raised by a Komen Affiliate remains in the community to fund local breast health education and breast cancer screening and treatment programs. Remaining net income (a minimum of 25 percent) supports Susan G. Komen for the Cure Award and Research Grant Program, which funds groundbreaking breast cancer research, meritorious awards and educational and scientific programs around the world.
